Police said a B-double travelling south on Botany Road veered into northbound lanes before hitting pedestrians, a power pole, bus stop and a building.

The driver was trapped at the scene but had been freed and taken to Royal Prince Alfred Hospital in a critical condition.

Reports suggest he possibly suffered a heart attack before the crash.

“He (the driver) was freed and taken to hospital, he may have had a medical episode, but that is yet to be confirmed,” NSW Police Superintendent Andrew Holland told reporters at the scene.

“An investigation will either rule that out or in. He’s obviously in shock.”

Superintendent Holland told radio station 2GB the incident “couldn’t be much worse”.

Of the injured, two people were taken to St Vincent’s Hospital with minor injuries and another was taken to RPA in a critical condition.

The condition of the two other injured pedestrians is unknown.

NSW Police said there was nothing to indicate the event was terror-related.

The crash brought down power lines and a dozen ambulance crews, firefighters and police are on the scene.

Botany Road remains closed in both directions between Bourke Street and Epsom Road.

Witness Robert Leash said the crash “freaked him out”.

“I didn’t know there were people there when all that happened,” he told reporters.

Commuters are warned to avoid the area.
