Death toll expected to rise as Melburnians struggle to grasp the horror that befell their city

Hundreds of mourners and community leaders have flocked to the centre of Melbourne to pay their respects to the victims of Friday's crazed car attack.

The Victorian premier, the police commissioner and the mayor of Melbourne were among the crowd still coming to terms with the shock, anger and sadness from the events that left four dead and more than two dozen men, women and children hospitalised.

Victoria Police commissioner Graham Ashton warned on Saturday morning the number of fatalities could rise with "grave fears" held for at least two victims, who are still in critical conditions.

The commissioner added the driver was under police guard in hospital, undergoing surgery for gunshot wounds.

Premier Daniel Andrews said he felt the "real anger" harboured by Melburnians.

"I share that anger and frustration," he said, telling reporters a full investigation into the incident would be carried out.

"We owe that to every single victim."

Today residents have flocked to the scene of the rampage to pay tribute to the victims.

Before his driving carnage through the city, Dimitrious Gargasoulas, also known as 'James' or 'Jimmy', allegedly stabbed his brother hours earlier and had posted disturbing messages on social media in the days before the rampage.

A three-month-old baby girl is fighting for life at the Royal Children's Hospital after police rushed her to the hospital following the frenzied attack.

The hospital on Twitter confirmed another victim, a two-year-old, is in a serious condition and a nine-year-old is stable.

There are seven patients still being cared for at the Alfred, with the hospital confirming in a statement there were three patients in a critical condition.

A further four patients were being cared for at the hospital, two in serious conditions and two reported to be stable.

St Vincent's Hospital confirmed the five patients still undergoing care there were all stable.

Three women, all in their 30s, were being treated for facial wounds and fractures and two men were being seen to for leg fractures, the hospital confirmed on Twitter Saturday morning.

St Vincent's has also corrected media reports that the man identified as the driver of the car involved in the Bourke Street attack, Gargasoulas, was being treated at the hospital.

The identities of the injured have not been released, nor have the identities of those killed in the attack.

There were 11 people under care at the Royal Melbourne Hospital on Friday evening, The Age reporting three remain in a critical condition.

Two patients are in a serious condition, four are stable and two have been discharged.

Victoria Police have so far only confirmed the ages and genders of the victims, with a 10-year-old girl, a 25-year-old man, a 33-year-old man and a 32-year-old man killed in the bloody onslaught.

Gargasoulas was known to police after a history of drug abuse and mental health issues, Victorian Police Commissioner Graham Ashton confirmed.
