Lars and Vivian died at the scene while Annabelle and Jessica were taken to hospital in critical conditions. Annabelle died three days after the crash.

The driver of the four-wheel-drive, Craig Whitall, also died in the fiery crash.

Jessica remains in a critical condition in St George Hospital two weeks after the tragedy while other family members prepare to say goodbye to her parents and sister in a funeral service on Wednesday.

In a funeral notice published in the Sydney Morning Herald, Lars, Vivian and Annabelle were described as a "dearly loved and cherished father, mother and sister to Jessica".

"Much loved daughter, son-in-law and granddaughter to Salvatore and Maria Catena Ponticello," the notice said.

"Lars, Vivian and Annabelle will be deeply missed by Vivian's brother Paul, his children along with many relatives and friends both near and far."

Tributes have poured in in for the family over the past two weeks. Annabelle's partner, Jordan Brohier, posted a heartbreaking tribute on his Facebook page following her death.

"I've never met such a kind, gentle soul and this world is at such a loss without this beautiful angel," he said.

"I know that the lord and her amazing parents will take care of my girl in heaven and I wish all the best to all of their family. They are all amazing people."

Jessica's remaining family are worried about how to tell her that her parents and sister have died if she does wake up.

"(Jessica) may wake up and, if she does wake up, is she going to be a different person?" James Randazzo, husband of Jessica's second cousin Cathy Randazzo, told News Corp Australia.

"If we get to that point then someone is going to have to break the news to her about the rest of her family."

Witnesses say they saw Mr Whitall's car cross into the path of oncoming traffic, colliding with the family's car head-on.

Mr Whitall had been jailed for driving while disqualified and had only recently got his licence back when the crash occurred.

A requiem mass for Lars, Vivian and Annabelle will be held in St Marys Catholic Church on Wednesday at 10.30am. Following the mass, a private family internment will take place.

In Lieu of flowers, family members are asking for donations can be made to Redkite.
