Johnson posted a photo of Connie on stage in front of 1400 medical graduates and wrote that "I had to wheel her offstage and into the wings, for the last time, as as soon as we were out of sight, we both buckled."

The Facebook post is full of affectionate in-jokes and nicknames.

He calls Connie his "bonehead sister" and "Little Miss Connie Cottonsocks" and refers to himself as "your ever grotty and very sad little brother, Sammy Seal."

Love Your Sister is the charity the pair set up together, and it has raised millions for cancer research.

"Stand big and tall, dear sister, for you have lifed a life to be proud of," Johnson writes.

"You've loved well, and you've been loved well, which is all that really matters in the end."
